NHS Surrey has commissioned Docobo Ltd to supply its new risk stratification tool, ArtemusICS™, that will enable GP practices in Surrey to better target their resources to deliver better outcomes for the 1.2 million people living in Surrey.

The tool takes patient data from both Primary and Secondary care and, through detailed analysis, will identify patients at risk of requiring future healthcare resources and who would likely benefit from a specific Long Term Condition intervention.   The data is presented in a convenient ‘dashboard’ format so that information can be easily accessed via secure web access by authorised staff at GP practices.    Data can be examined based on groups of patients, such as by practice or locality, or by clinical condition.  Each patient is allocated a ‘risk score’ which predicts their risk of a hospital admission in the next year.  Their GP can view this data and use it to direct their resources more effectively. This enables them toprovide more targeted personalised care and will help to identify and treat health problems earlier. Furthermore, the patients’ risk scores can be monitored regularly to examine the effect effectiveness of treatments and interventions.
